What do I need to do before my first visit?

You will be directed to our web site to download and complete several forms that you will bring with you at the first visit. If you do not have the internet, you will be receiving a packet in the mail prior to your visit. It is very important that these forms be completed in their entirety and as comprehensively as possible. We will be using this during your first physician evaluation. Please bring this with you on the day of your first visit. If you have not completed the paperwork when you arrive, your first evaluation and BEST test will have to be rescheduled.

Please bring all nutritional supplements you may be taking with you on the first visit. Also please do not take any supplement for 24 hours prior to your first visit. Please continue to take all prescription medications as usual.

If your primary care physician has done any blood tests or other tests, please obtain a copy and bring them with you to the first visit.

We never double book patients as many other physicians and practitioners do. This time is specifically set aside for you. As a result we will require a $100 deposit or credit card to secure the visit. No monies will be debited until the day of your visit. We understand that situations arise and an appointment may be missed. We do require that all cancellations must be received a full 24 hours in advance. Unexcused cancellations will be debited $100.

Day Two

On day two, you will meet with your doctor and review all that we have found in our initial testing. We call this a "Review of Findings Evaluation" It is here that we will present to you a therapeutic program or "game plan" if you will. We will discuss time frames, therapies, additional diagnostic tests if needed, etc. Typically this program will encompass approximately 6 months. We find that if a patient commits to a prearranged program, the success rate is much higher.

If we cannot complete both the initial evaluation and the BEST test on the same day, the BEST test will be scheduled for a second day and the review of findings evaluation a third day.
